Home Care provides a service continuum to functionally impaired, low income adults 60 years of age and older, who are at risk of premature nursing home placement and reside with a full time caregiver.  

This program provides:

• Case Aide
• Case Management
• Basic Subsidy Stipend to Care Givers
• Subsidies for Specialized Services to Assist Caregivers.
