Groups wishing to adopt a street in a participating city must first submit a Group Application Form. Applications are processed on a first-come, first-serve basis.

Forms for Current Adopt-A-Street Groups:
An Event Completion Form must be filled out and submitted by the Group Leader after each cleanup conducted on your adopted street. Release and Waiver Forms are signed by each volunteer and submitted with the Event Completion Form. Both forms must be submitted for an event to be credited toward your group's four cleanups per year.
