Community Care provides a service continuum to functionally impaired, adults 60 years of age and older who are at risk of premature nursing home placement. Sliding scale co-payment determined by income.

Services may include:

• Case Management​
  
• Case Aide
• Homemaking

• Personal Care

• Respite Care
• Emergency Alert Response
• Adult Day Care
• Monthly Caregiver Stipend
  
• Reimbursement for Consumables and Medical Supplies

Eligibility qualification criteria for the services provided by this program are determined by factors such as age and availability of funding.​​
