Elderly Services
The Elderly and Veterans Services Division, utilizing a holistic service delivery approach, empowers elders, veterans of all ages and their families/caregivers to improve their quality of life through supports that promote self-sufficiency and aging in place. Division services include Customer Relations and Senior Services, Community Care for the Elderly, and Behavioral Health.

Provides a service continuum to functionally impaired adults 60 years of age and older who are at risk of premature nursing home placement.
Services may include:
- Case Management
- Homemaking
- Personal Care
- Respite Care
- Emergency Alert Response
- Adult Day Care
- Monthly Caregiver Stipend
- Consumables and Medical Supplies
Sliding scale co-payment determined by income.
Provides assistance to veterans, their widows, dependents and dependent survivors regardless of age or other status. Services are focused on attainment and/or maintenance of self-sufficiency.
Services Include:
- Outreach
- Information, Referral and Linkage
- Benefits and Claims Processing

Serves as the "Welcoming Open Door" for accessing all Division services including:
Services may include:
- Receives all customers to simplify appropriate connections
- Facilitates prompt screening to identify and prioritize customer needs
- Coordinates individualized and appropriate services
- Follows up to ensure timely and integrated quality response by Division and external agencies

EHFI – Elder Housing First Program
Home Care for the Elderly (HCE)
Provides a service continuum to functionally impaired, low income adults 60 years of age and older, who are at risk of premature nursing home placement and reside with a full time caregiver.
Services may include:
- Case Management
- Case Aide
- Subsidies for Specialized Services to Assist Caregivers
Alzheimer ’s Disease Initiative (ADI) Program
Alzheimer’s Disease Initiative provides a service continuum to functionally impaired residents, 60 years of age and older who have received a diagnosis of Alzheimer’s or dementia related diseases who may be are at risk of premature nursing home placement. Sliding scale co-payment determined by income.
Services may include:
- Case Management
- In-Home Respite Care
- Care Giver Support and Training
- Consumables and Medical Supplies
Eligibility qualification criteria for the services provided by this program are determined by factors such as age, diagnosis and availability of funding.
